Please fill out the radio assignment form for each radio in your group. Make sure to include the following:
- Name: The name of the person who uses the radio.
- Shop: The department or team responsible for the radio. For example, Zone Maintenance or Construction.
- Radio Alias: This is the number we commonly use to call each other. For example, (FS243).
- Radio ID: This is an eight digit number beginning with four zeros. To save time, you may include only the last four digits when filling out the form.
- Serial Number(S/N): This can be found on the back of the radio, under the battery.
- Signatures & date.
